Affidavit For Purposes of Inclusion Into an Insurance Scheme
This Affidavit is sworn by parties who are married to first confirm that they are legally married to each other and second, in support of an application to have a spouse or child added to a medical insurance cover e.g NHIF and any other insurance scheme.
Please note that this Affidavit must be commissioned by an advocate of the High Court of Kenya and an extra charge might apply.
